My approach: in summer, I skip the insert entirely. The cover over your percale sheets is usually enough for a warm night, and most of us run too hot for a filled duvet anyway. In cooler months, cotton or wool blanket layers better than a duvet insert — it gives you more control as your temperature swings through the night.
The matching pillowcases aren't just aesthetic. Breathable fabric on all sides is the point.
